We operate the parts of a CCM and RPM program that independent practices rarely have staff hours for: patient identification and consent, device logistics, daily review of transmitted readings, monthly telephonic care-management calls, escalation per your protocols, and documentation your billing team can work from.
Every interaction is remote, telephonic, and platform-based, performed by NexCare's licensed nurses under the practice's general supervision. Your physicians define the escalation pathways and retain all medical decision-making and all billing.
